HCSG Employees
Contact your account manager to enroll in healthcare benefits.
You are eligible for this health plan if:
- You are considered a full-time employee by HCSG,
- You are a member of SEIU Local 503.
You’ll need to have some information with you to enroll:
- The name, birthdate, and Social Security number for everyone listed on your application (for example, your spouse and/or dependents)
- If you or your dependents will also be covered by another health insurance plan, you will need to provide details on those plans as well
- Please remember to select “HCSG” for your employer.
